Preparing for OSCE Under Realistic Conditions

Most OSCE preparation rehearses content. The station tests performance under time pressure with an examiner watching — which is a different skill, and it is trainable.

Most students prepare for an OSCE by revising content — reading through conditions, memorising examination sequences, rehearsing the structure of a history.

That preparation is necessary and it addresses the wrong failure. Students who underperform in OSCEs are rarely students who did not know the material. They are students who knew it and could not produce it in eight minutes, in a room, with someone watching and a clock running.

Performance under those conditions is a distinct skill from knowledge, and it is trainable — but only by practising under conditions that resemble them.

What the station is actually testing

An OSCE station compresses a clinical encounter into a short, observed, standardised window. That compression tests several things simultaneously:

Whether you can start. A surprising number of marks are lost in the first thirty seconds, spent orienting rather than acting.

Whether you can prioritise. There is not time to do everything. The station rewards doing the things that matter for this presentation, in a defensible order.

Whether you can be systematic under pressure. Structure is what survives when working memory is loaded. Students who rely on recall rather than a system tend to omit things they demonstrably know.

Whether you communicate while doing. Explaining to the patient, and to the examiner, while performing.

Whether you recognise limits. Knowing when to escalate is frequently a marked item and frequently forgotten.

Notice that only the middle of this is knowledge. The rest is performance.

The specific failures, and what fixes them

Freezing at the start

The commonest and most fixable. A student reads the stem, and the first thirty seconds go on deciding what to do.

The fix is a rehearsed opening that fits any station of a given type — introduce, confirm identity, state purpose, ask an opening question. Automatic, so that the thinking capacity goes to the case rather than to the choreography.

Omission under load

Under pressure, people forget steps they know perfectly. Not a knowledge failure — a working-memory failure.

The fix is having a structure rather than recall. A student working through a fixed sequence does not have to remember what comes next, which frees capacity for the content of the case.

Running out of time

Usually caused by spending too long early — an exhaustive history that leaves no time for examination or management, both of which carry marks.

The fix is practising with the actual clock. Students consistently misjudge elapsed time, and the only correction is repeated exposure to the real duration.

Silence while thinking

Marks are frequently attached to communication. A student reasoning silently may be reasoning well and demonstrating nothing.

The fix is practising narration until it is comfortable, because it feels unnatural at first and becomes automatic with repetition.

Why repetition is the binding constraint

Every fix above responds to repetition rather than to reading. And repetition is the thing OSCE preparation is usually short of.

Practising with peers is valuable and limited: a peer cannot simulate an unfamiliar presentation convincingly, and both students already know the case. Practising with faculty is better and scarce — the same observer-hours problem that constrains competency-based education generally.

The result is that most students arrive having rehearsed content extensively and performance rarely.

This is where simulation has a specific role. Not because it replicates an OSCE examiner, but because it provides volume: many encounters, unfamiliar presentations, under time pressure, with structured feedback afterwards. A student who has managed a hundred encounters has automated the opening, the structure and the pacing — which is exactly what the station tests beyond knowledge.

Practising the right things

If preparation time is limited, it is better spent on the failures above than on more content.

Concretely:

  • Rehearse openings until they are automatic
  • Practise with a timer, always, because time perception is the thing that misleads most
  • Practise narrating while doing
  • Practise unfamiliar presentations, not the ones you feel confident about — the confident ones are not where marks are lost
  • Practise the ending — summarising, safety-netting, stating when you would escalate

That last one is disproportionately valuable, because it is both frequently marked and frequently omitted when the clock runs down.

Two things it does not do.

It is not an OSCE simulator. An OSCE is a standardised station with a trained examiner and a marking scheme. Simulation provides practice, not a replica of the assessment format.

It does not replace rotations or faculty teaching. It changes how much practice a student can accumulate, so that scarce supervision is spent where judgement is needed.

What it does address is the volume problem. The reasoning behind assessing across nine separate dimensions rather than a single mark is in how clinical reasoning is actually assessed — and for OSCE preparation specifically it means a student learns which dimension they are weak in, rather than that they scored 68.

Frequently asked questions

How early should preparation start?

Performance skills develop with repetition over months, not in a fortnight. Content revision compresses; automating an opening and calibrating time sense does not.

Is practising with peers useful?

Yes, with a limitation: both of you know the case, so the element of encountering something unfamiliar is absent. Combine peer practice with unfamiliar presentations from another source.

What if I know the material but perform badly?

That is the specific problem this article describes, and it is the more tractable one. Work on openings, structure, timing and narration rather than on more content.

How much does time pressure really matter?

More than most students expect. It changes what you can recall and how systematically you work, which is why practising without a clock produces a misleading sense of readiness.

Does simulation practice transfer to a real station?

The performance components — opening, structure, pacing, narration — are the same skills. The examiner interaction is not replicated. Treat it as building the parts that are trainable at volume.
